How is this a monopoly?
The firestone department of transportation basically does the following;
Taxi services, The FDOT uses taxis to transport people for free.
Towing Services The FDOT uses tows to tow cars.
Roadside assistance The FDOT uses cones to reroute cars in the scene of a situation. And the FDOT uses stuff such as tire pumps to repair vehicles.
How is this a problem?

People are unable to make taxi services due to the fact that FDOT has free taxi rides and they basically control the taxi industry. You are unable to compete unless you want to make 0% profit by also having your taxi rides free. In real life, taxi services are done by private companies, not FDOT.

The Department of Health currently owns all hospitals in the state of firestone. This is a monopoly because private companies would be unable to create their own hospital. Also having one department control all hospitals could lead to that department raising costs of treatment in a hospital for money advantage because there is no competition.
What should the DoH department do instead?
The DoH department should focus on regulation of certifications on being a Registered Nurse(RN), Nurse Practitioner (NP), Physician Assistant, (PA), Physician (MD or DO) and other specialities. The DoH should teach these certifications and require these certifications to work at a hospital.
The DOH Department should also focus on research of medical issues and support private companies to do these researches as well. The DoH department would not own the hospitals but however regulate them and do general checks to make sure that everyone working in that hospital has certifications to act as a nurse, doctor, speciality, etc.
